​You are about to enter the scholarship process for the Cizik School of Nursing at UTHealth. The School currently offers both merit and need-based scholarships. In order to be considered for both, all students are encouraged to complete this application as well as the Free Application for Federal Student Aid (FAFSA). 

Students who have been accepted for admission are eligible to submit the scholarship application. 

Please note that the application will ask for you to upload a personal statement that addresses your area of academic interest, future nursing aspirations, and how receiving a scholarship will benefit you.

Personal statements must meet these requirements: 
  • Have your name in the upper right-hand corner
  • Be in either PDF or Word format
  • Cannot be longer than one page in length (double-spaced, 12 pt New Times Roman font, 1" margins),
  • Name the document with your last and first name only.
