In compliance with Governor Abbott's Executive Order GA-48, UTHealth Houston employees who plan to travel for personal reasons to a foreign adversary country as defined by federal law - currently China (including Hong Kong and Macau); Russia; Iran; North Korea; Cuba; and Venezuela (Maduro Regime) - must notify the University prior to travel and, upon return, must submit a post-travel brief outlining the details of the trip, including the dates and purpose of travel. This form is for the pre-travel notification.
